SCHEDULE 1
 
  NOTICE OF INTENTION TO EMPLOY A CHILD
     1. A notice of intention to employ a child shall contain the information set out in paragraph 2 of this Schedule ("the relevant information") and shall be accompanied by the documents listed in paragraphe 3 of this Schedule ("the appropriate documents").
 
 
Relevant information
     2. The information which shall be included in a notice of intention to employ a child includes-
 
 
    (a) the prospective employer's name and address and the name and position of the person who shall be responsible for supervising the child's work;
 
    (b) the name, address and date of birth of the child;
 
    (c) the type of work which the child is to undertake and the place in which the work is to be undertaken;
 
    (d) the date on which the work is due to begin;
 
    (e) the days on which the child is to work, the times at which the child will start and finish work each day and the duration of any rest periods during each day;
 
    (f) if the child is to be paid, the rate of remuneration or, as the case may be, the method of calculating any remuneration, and the intervals at which such remuneration is to be paid;
 
    (g) the child's entitlement to holidays;
 
    (h) where the work is not intended to be permanent, the period for which it is expected to continue or, if it is for a fixed term, the date on which it will end;
 
    (i) the name and address of the child's school;
and any other information which the relevant authority may reasonably require.
 
 
Appropriate documents
     3. The documents which shall be supplied with a notice of intention to employ a child are-
 
 
    (a) the prospective employer's liability insurance policy certificate; and
 
    (b) the certificate of fitness given under section 11 of this Act.
