  National Institute for Clinical Excellence: Dr Tim Crayford, Deputy Director of Public Health and Chairman, Prescribing Committee, and Helen Marlow, Pharmaceutical Adviser, Croydon Health Authority, Dr Deirdre Cunningham, Director of Public Health and Health Systems, Lambeth, Southwark and Lewisham Health Authority, and Mr David Walker, Director of Public Health, Newcastle and North Tyneside Health Authority, were examined.

  Mr Eddie Gray, General Manager and Senior Vice-President, GlaxoSmithKline, Mr Hugh McKinney, Director, and Mr David Campbell-Morrison, Director, Campaign for Effective and Rational Treatment, Dr Trevor Jones, Director General, Mr Edgar Fullagar, President, and Dr John Patterson, President-elect, Association of the British Pharmaceutical Industry, were examined.
