Memorandum by Addenbrooke's NHS Trust Maternity Services (MS 58)

Rosie Hospital Profile

  The Maternity Service is part of Addenbrooke's NHS Trust and is based in the Rosie Hospital, on the South side of Cambridge City Centre. It is a purpose built Maternity Unit offering a combination of inpatient and outpatient facilities. The accommodation was built in 1983, is pleasant and functional and the overall impression supported by both users and local review is of a popular and efficient service.

  The Rosie Hospital provides maternity services to the local and regional populations of Cambridgeshire and Huntingdon, North Essex and East and North Hertfordshire, Suffolk and Bedfordshire.It also functions as a tertiary referral centre accepting referrals from surrounding Trusts for women and babies with complex medical problems. Currently undertaking 4,500 deliveries annually, with 35% of the referrals originating from outside Cambridgeshire.

  Antenatal care is provided both in the hospital and community and shared between Obstetricians, Midwives and General Practitioners. Women are offered full antenatal screening services and a smaller number offered antenatal diagnostic services. There are facilities for day care surveillance and inpatient treatment. Community based staff have access to these facilities, and to the advice of the hospital based team.

  Intrapartum care is hospital based, for the majority of women. In 2002, 3.95% of women in the Cambridge Health District chose to have a home birth attended by community-based midwives.

  Postnatal care is largely shared between the hospital and the community. The average length of stay (LOS) in hospital for 1999 was 2.77, for 2001, 2.47 days and 1.81 by the end of 2002. LOS has decreased during this period of time as a change in policy, regarding early discharge, was introduced in 1999. Following transfer home the women are supported postnatally by their community midwife for up to 28 days. The GP then reviews the woman and her baby between six to eight weeks.

  Midwives are the largest group of qualified staff at the Rosie, undertaking 59.6% of deliveries in 2002 and present at 100% of births. Midwives work in both hospital and community, the majority, 75%, working primarily in the hospital setting. The remainder work primarily in the community but support the hospital when necessary.

  Supervisors of Midwives—there are 15 Supervisors of Midwives within the Rosie, who are currently working to try and raise the profile of Supervision. The group of Supervisors includes a combination of both Managers and Clinicians.

  Consultant Obstetricians—there are 10 Consultant Obstetricians at the Rosie who each plan and oversee the care of a caseload of women. Doctors in training provided the major part of direct care, however the emphasis is shifting to a Consultant delivered service.

  Senior Consultant Anaesthetic presence is available in both the Delivery Unit and Obstetric Theatre throughout the week, providing consistent and excellent care. Twenty-four hour "on call" cover is also provided. There is a comprehensive pain relief service, anaesthetic support for the elective and emergency theatre activity, and active involvement with Obstetricians in the management of women requiring "high dependency care".

  The service benefits from the close proximity of the Neonatal Intensive Care Unit and the support of the Neonatology team of medical and nursing staff. Joint planning for care between Obstetricians and Neonatalogists is well developed, as are the discussions around the tertiary part of the service where both the mother and baby are transferred to the Unit from the wider catchment area.

  The Maternity Service is supported by other key clinical specialities such as Neonatology, Obstetric Anaesthesia, Radiology, Pathology, Genetics and Professions Allied to Medicine.

  The Maternity Service supports the training and education of medical, midwifery, nursing and other groups of staff and the qualified staff are involved in development of training programmes, examinations and assessments.

THE UNIT

  Rosie Outpatients: Obstetric, Gynaecology, Family Planning and Genetics O/P clinics.

  Rosie Scan Department: Obstetric and Gynaecology scans.

  Daphne Ward: Fetal Day Assessment; Assessment of pregnancy (16/40); Emergency gynaecology; Early Pregnancy, Reproductive medicine.

  Delivery Unit: 14 Delivery Rooms, with a Birthing room (incl Pool) and High Dependency Unit being currently developed.

  Rosie Theatres: 2 x obstetric theatres adjoining Delivery Unit.

  Ante/Postnatal Wards: Christine Ward and Lady Mary Ward.

  Community Teams: four teams serving the population of Cambridge Health Authority.

  Neonatal Intensive Care Unit—with nursery and transitional ward.
