Memorandum by Trafford Healthcare NHS Trust (MS 73)

SUMMARY

  Trafford General Hospital is a District General Hospital situated in the North of Trafford. The Maternity Service moved into a purpose-built building in 1992. The Unit consists of 24 beds providing in-patient care for ante-natal and post-natal women. There are five birthing rooms, an Obstetric theatre, a Special Care Baby Unit with eight cots and an Ante-Natal Assessment Unit. Currently awaiting the new build for a Bereavement/Family area. Adjacent to the Maternity Unit is the Ante-Natal Clinic and Gynaecological Unit.

1.   Staffing

  There are four Consultant Obstetrician/Gynaecologists (two of which are maximum part time) who participate in 1:4 on call. There are five middle grade doctors who participate in 1:5 on call and five SHOs who participate in 1:5 on call.

  Staffing levels are frequently depleted, especially when sickness, annual leave and study leave occur. However, recruitment is usually successful.

2.   Midwifery

  Head of Midwifery/Service Manager for Obstetrics, Gynaecology, Well Adults & Sexual Health and Child Health Directorates.

  3 x "H" grade Midwifery managers—(Community Midwifery, Labour Ward and In-patient Unit).

  62.16 whole-time equivalent Midwives, (Community and Hospital).

   One Infant Feeding Co-ordinator.

  Two midwives who provide care to young parents (Teenage Pregnancy).

  Two Community Midwives—who provide education and support to women with drug/alcohol dependency, smoking cessation.

  One Sure Start Midwife for Partington area.

  Support staff of Heath Care Assistants, Ward Clerks, Volunteers.

  Recruitment in midwifery has only become a problem within the last year, currently vacancies now running at approx 8%.

  The Trust offers Flexible Working: long shifts, part-time, job-share posts.

3.   Models of Care offered

   Consultant Care—all care provided by Hospital Consultants.

  Shared Care—(booked under a Consultant, but the majority of care provided by midwives).

  Domino care—Booked under a Consultant but care provided by Community Midwives, including intrapartum care and usually six-hour stay post delivery.

  Midwifery Led Care—(women assessed at low risk of complicated pregnancy plus labour who choose to receive all their antenatal, intrapartum and postnatal care from Midwives) currently being developed and to be implemented 1 April 2003.

  Women in labour of less than 32 weeks gestation are usually transferred to a Regional unit with NNIC eg Saint Mary's, Manchester or Hope Hospital, Salford.

  The Obstetric service is supported by 24 hour cover of Paediatric and Anaesthetic teams.

COLLECTION OF DATA

  CESDI provides a comprehensive review of data which is comparative between Units in the North West.

  Hospital Computer system is used to generate obstetric discharge letters, collects varied obstetric data.

  Manual collection of data for monthly comparative statistics.

  Please note: Currently approximately 2,500 births to Trafford residents per year of which approximately 1,500 deliver in Trafford, about 850 deliver in Wythenshawe Hospital and 150 per year deliver in Saint Mary's, Manchester. However, most antenatal and postnatal care is provided by Trafford Midwives.
