(1 )Cause of death was defined using the International Classification of Diseases, Eighth Revision (ICD-8) for the years 1976-78, Ninth Revision (ICD-9) for the years 1979 to 2000, and Tenth Revision (ICD-10) for 2001 onwards. The introduction of ICD-9 in 1979 and ICD-10 in 2001 means that the numbers of deaths from specific causes are not completely comparable across the whole period. The ICD codes for each cancer site are given in the following table.
